Is the ¥20,000 per year or for the whole degree?

Per year. It covers tuition, the hostel, health insurance and the visa extension for that year. The ¥2,000 service charge is paid once.

Can I sit PLAB in the UK or AMC in Australia afterwards?

CSB's information is that graduates are eligible for PLAB (UK), AMC (Australia) and the ECFMG route to the USMLE, through the university's World Directory of Medical Schools listing. Each body sets its own rules and changes them, so check with it directly before you plan around it.
